What is a Statutory Declaration of Progress Payment Distribution?
A Statutory Declaration of Progress Payment Distribution by Contractor is a sworn statement for use by a contractor or subcontractor as a condition of receiving payment for either the second and subsequent applications for progress payment or the release of holdback funds. A contractor or subcontractor declares that all amounts payable by them as a result of their receipt of a specified progress payment have been made.
The statutory declaration is available in two formats: CCDC 9A for prime contractors; and CCDC 9B for subcontractors. Both of these are available for download for free from the Canadian Construction Documents Committee’s (CCDC) website. The CCDC develops, produces and reviews standard construction contracts, forms and guides.

Royal Architectural Institute of Canada (Architecture Canada).
How do I complete a Statutory Declaration of Progress Payment Distribution?
Completing the statutory declaration is fairly straightforward. Once you fill in the details on the form, you will need to take it to a notary public or a commissioner for taking oaths and affidavits to complete the declaration part of the form. The notary or commissioner will review your identification to verify your identity. Then you will be asked to sign the document - note that the document must be signed in the presence of the notary or commissioner.
