• Affidavit Template: An affidavit is a sworn statement of facts

• Joint Affidavit Template: A joint affidavit is an affidavit that is sworn to and signed by two people

• Affidavit of Identity (One and the Same Person) Template: An affidavit of identity is used to demonstrate that different names on different documents refer to the same person

• Statutory Declaration Template: A statutory declaration is written summary of facts which a person solemnly states to be true

• Statutory Declaration of Marital Status Template: A statutory declaration of marital status is required if you are applying for a statement in lieu of certificate of non-impediment to marriage abroad

• Sworn Statement for Registrant Identification Number Template: Use this template to create a sworn statement to register a car on behalf of a business

• Declaration of Pleasure Craft Ownership: Use this template to create a pleasure craft ownership declaration for a pleasure craft licensing application
